Specialised Support Coordination

Specialised Support Coordination for complex needs

When life is complicated, with multiple services, higher risks or a crisis to work through, specialised support coordination brings everything together. We take a solutions-focused approach and keep every provider on the same page.

Housing and crisis support

When accommodation breaks down or a crisis hits, we act quickly to stabilise things and find a way forward.

Clear records and reports to support your plan reviews and any change-of-circumstances requests.

Questions

Specialised coordination FAQs

See all FAQs →
How is this different from standard support coordination?

Specialised support coordination is for situations with higher complexity or risk. It focuses on resolving barriers and coordinating many services at once, rather than everyday plan use.

How do I get specialised support coordination in my plan?

The NDIA decides the level of coordination funded in your plan. If your needs have become more complex, we can talk with you about requesting a plan review.

Can you help with a hospital discharge?

Yes. Hospital social workers and discharge teams can refer directly through our referral form or by calling 0478 532 025.
